Spectrum Care LLC – Notary Services Policy 1. Scope of Services Spectrum Care LLC provides professional notary public services, including but not limited to: Acknowledgments Jurats Oaths & Affirmations Power of Attorney Loan Signing & Real Estate Documents Affidavits Consent to Travel Letters Healthcare Directives Services are available via mobile notary visits by appointment only. 2. Identification Requirements All signers must present valid, government-issued photo identification at the time of the notarization. Acceptable forms include: State-issued driver’s license or ID U.S. Passport Military ID Permanent Resident Card The ID must be current or issued within the past 5 years if expired (based on Iowa law or the applicable state’s laws where the service is performed). 3. Refusal of Service The notary reserves the right to decline service if: The signer cannot produce acceptable ID The signer appears unwilling, unaware, or under duress The document is incomplete or contains blank spaces There is a suspected conflict of interest Notarization requested is outside the scope of lawful practice 4. Fees & Payment Fees are based on: The number of notarial acts performed Travel distance Time outside normal business hours (after-hours or weekend appointments) All fees will be disclosed and agreed upon before services are rendered. Payment is due when appointment is set. We accept all major credit cards. We do NOT accept cash as a form of payment. 5. Appointment & Cancellations Appointments must be scheduled in advance. A minimum of 2 hours’ notice is required for cancellation or rescheduling. A cancellation fee may apply if: The notary has already traveled to the appointment location The client is a no-show after 15 minutes Required documents or ID are not available 6. Mobile Notary Coverage Area We provide mobile notary services throughout Polk County Iowa and surrounding Counties. Travel fees apply based on distance and will be quoted at the time of booking. 7.
